About Partners in Care
Partners in Care is a fulfillment of a dream that Kids Crossing’s founder has had for many years.
Jan and Larry Faubion founded Kids Crossing in 1992 in response to the increasing demand for placements of special needs and medically fragile children in the foster care system.
Jan and Larry provided foster care to young people for over 40 years. Two of these children aged out of the foster care system but needed services throughout their lives, so Jan and Larry began a new adventure in the adult field.
In 1994 the Faubions turned Kids Crossing into a non-profit organization. The agency continues to be run by a Board of Directors today. The Board of Directors is made up of community leaders, including Jan Faubion, who determines the mission of Kids Crossing, sets the annual budget, and ensures the organization remains financially stable. During one of the Board meetings, Jan presented a desire to provide adult services in conjunction with children’s services.
In 2017, Kids Crossing began the process of becoming a Program Approved Service Agency (PASA). In early 2018, Partners in Care became a reality. Partners in Care envisions a partnership with individuals, their families and their teams to see each person achieve their fullest potential toward independence. We believe in creating a safe and accepting atmosphere where individuals will be comfortable to be themselves, express themselves, and strive for new goals.
Partners in Care works to create an Individual Residential Services and Supports (IRSS) program that provides quality services to adults with intellectual/developmental disabilities. We train host home providers to care for adult clients aged 18 and older, creating an enriching and supportive physical and emotional environment. The host home provider is responsible for meeting the daily living needs of clients, as well as acting therapeutically with clients to bring about change in problem behaviors. Host home providers use their personal knowledge and expertise, follow treatment plans, and consult with Partners in Care staff and other professionals as needed to affect change in behaviors. The host home providers are responsible for creating a broad social network for clients and teaching clients to make use of those supports.
Kids Crossing Child Placement Agency provides foster care services to children with intellectual/developmental disabilities. These children age out of the foster care system; opening our adult program has allowed the Agency to maintain stability for these individuals by allowing them to remain in the same home when they transition to adult care. It also allows the agency to serve the intellectual/developmental disability population in our area with the same level of quality services we have been providing for children since 1992. It is our desire to improve the quality of life for an at-risk population by providing the most comprehensive, nurturing environment for all individuals. Partners in Care stands for high quality, focusing on meeting individual needs, and striving to raise community standards with our comprehensive habilitation plans.
